How Do You Amend Oklahoma Birth Certificates?
Sometimes it’s necessary to amend a birth certificate to correct an error or to reflect an adoption. In Oklahoma, an amendment can be requested by the subject of the birth certificate, or their parent, spouse or lawyer, provided they have the proper information and identification. Someone else, such as an adoptive parent or guardian can also request an amendment. Minor changes can be made by presenting the proper documentation, but major changes, such as significantly changing the birth date, require administrative review and possibly a court order.
