How do we keep track of patients referred for audiological evaluation at JTC?
Be sure to immediately fax referrals to JTC. The BSC Champion at your facility should keep the faxed results in a separate file that is reviewed regularly. Alternatively or in addition, referrals (and rescreens) can be highlighted in the patient Log Book and reviewed regularly. JTC is monitoring all BSC referrals, contacting families for appointments, and faxing reports to BSC providers. If a family cannot be contacted or does not show for a scheduled appointment, a notification letter is faxed to the provider and the medical home should follow up directly with the family to facilitate appropriate follow through.
