How do pharmacies become MADAP participants?
Pharmacies currently submitting claims with the Maryland Medical Assistance Program (Medicaid) are automatically eligible to participate in the MADAP program. Call MADAP at (410)767-6535 and ask to be added to the pharmacy list. Please be prepared to give your Medicaid provider number and your federal tax identification number. If pharmacies are not a Maryland Medicaid provider and would like to participate in MADAP, please contact Maryland Pharmacy Network Enrollment at (410)767-5340 and First Health Services Provider Relations at (804)965-7729.
