If your patient is scheduled to see a specialist, we will forward all of the necessary material directly to the specialist’s office. Their records will be sent out that day to ensure that their specialist has adequate time to review all of the material before your patient’s scheduled appointment. Patients who would like their records released should call 570-586-4400 and ask to speak with Karen. She will need the following information: name of the specialist; date of appointment; name of hospital or institution; name of individual to whom records should be directed; mailing address; and phone and fax number.
