• You will need to fill out the lending form. No faxed or e-mailed requests will be accepted. • If you have never submitted requests to FUG, you will be required to fill out the Vendor Application in addition to the W-9 Form (national) or W-8BEN Form form (international) BEFORE we can fill your request. Please fax the completed forms to 352-392-7598, or e-mail to Lending.
