How do I view the form with another version of Adobe Acrobat Reader (other than version 8)?
If you are using a version of Adobe Acrobat Reader other than version 8, you must first download and save the form to your computer for the form to work properly. Next, open your Adobe Acrobat Reader software and select “File” from the top menu, then select “Open” from the drop-down menu. A new window will open from which you can retrieve the form you saved on your computer.
