How do I use the Express Scripts home delivery program?
Complete the Mail Order Form by providing the required member and payment information, attach your original written prescription(s) and mail to the address on the form. Members can expect to receive their first home delivery prescription(s) within 14 days of the date the prescription was mailed to Express Scripts. Alternatively, you can print the Fax Order Form, take it to your doctor and have your doctor fax your prescriptions directly to Express Scripts. You may request forms and envelopes for the home delivery through your local human resources personnel or by calling Express Scripts customer service at 1-866-291-2607.
