if the custom map is for example mp_pk_harbor you upload to /usermaps/mp_pk_harbor/ from the cod4 root folder example: cod411077/usermaps/mp_pk_harbor the .ff and .iwd files that belong to the map is placed in that folder after this is done you must restart your server.
