Odometer information required on all motor vehicles less than ten (10) years old, with buyer’s signature, printed name, and date. • Part 3 (Purchaser’s Certification and Application) must be completed and signed by new owner with tax exemption and lienholder information, if applicable. • If vehicle is less than nine (9) years old SFN 18609 Damage Disclosure Statement must be completed by the seller. • Title transfer fee is $5.
