Mail the completed form to the Nurse Aide Registry in the state where you are currently listed. You can find the address and phone number of each state’s nurse aide registry Nurse Aide Registries (by state) (PDF, 68 KB) or via http://www.pearsonvue.com/ (exit DHS): • Select “Registry Services” under “Find Other Services” • Select “Wisconsin Nurse Aides” • Select “Nurse Aide Registries (by state)” Nurse aide registry personnel in that state must complete Part II of the application to verify your status in that state.
