How do I transfer my status from another states Nurse Aide Registry to the Wisconsin Nurse Aide Registry?
You may get the Registry Out-of-State Application form ( exit DHS; PDF, 112 KB) via http://www.pearsonvue.com/ (exit DHS): • Select “Registry Services” under “Find Other Services” • Select “Wisconsin Nurse Aides” • Select “Registry Out-of-State Application” Complete Part I of the application and attach verification of your personal identity, including: • Name, • Date of birth, and • Social Security Number. Mail the completed form to the Nurse Aide Registry in the state where you are currently listed. You can find the address and phone number of each state’s nurse aide registry Nurse Aide Registries (by state) (PDF, 68 KB) or via http://www.pearsonvue.com/ (exit DHS): • Select “Registry Services” under “Find Other Services” • Select “Wisconsin Nurse Aides” • Select “Nurse Aide Registries (by state)” Nurse aide registry personnel in that state must complete Part II of the application to verify your status in that state. Only nurse aides who completed a training program of 120 hours or more are eligible to transfer to Wisconsin.
