How do I transfer my mail-order prescriptions to Sharp Health Plan’s mail-order pharmacy provider?
To transfer your mail-order prescriptions, your doctor will first need to write a new prescription for the medications you wish to transfer. You then complete the Rx Mail Order Form included in your Sharp Health Plan Enrollment Kit. To complete your request for mail-order prescriptions, simply send the completed form and your prescriptions to WellPartner at the address on the form.
