How do I submit a request to Bayer Diabetes Care for meters and/or test strips?
Please choose the Charitable Contributions form and select “diabetes” as the Therapeutic area. In the dollar amount field, please enter $1.00 unless you are requesting additional funding for a program or an event (in that case, please enter a specific dollar amount). Diabetes Camp Directors: Please note that Bayer Diabetes Care is not providing monetary donations to camps in 2009. Camp directors must also specify the total anticipated number of campers in the “event attendees” field. Total turnaround time for product donation requests for diabetes camps is 2 weeks. Donation requests for other organization types will be reviewed in the usual 8-10 week timeframe.
