How do I submit a request for product to use at an educational activity?
You must submit a complete grant request, which includes all the required documentation. In addition, please be sure to request product by clicking “yes” in the box that corresponds to the question “Will you be requesting in-kind product for this program?” This box can be found within the Budget section of the grant application. Within this section, you will be asked to specify the type and amount of product you are seeking. If you are also requesting a monetary contribution, you will be required to complete the entire budget section. If you are only requesting product, you are not required to submit a comprehensive activity budget. Lastly, you must complete and submit the Product Request Form. This form is available for download as you complete the grant application. Please be sure to select the correct tab (therapeutic vs. cosmetic) when filling out the form.
