Go to the USAMMA homepage (http://www.usamma.army.mil) and select Document Reference, then Medical/Dental Product Quality Deficiency Report (formally SF380). This then leads you to several options to either view the information paper, view the Army Regulation, Instructions For Filling Out the Medical / Dental Product Quality Deficiency Report, or Medical / Dental Product Quality Deficiency Report.
