SAM has invited your company to participate in the Corporate Sustainability Assessment for the Dow Jones Sustainability Indexes. If you have not received the invitation package please contact SAM at index@sam-group.com or phone +41 44 653 10 30. Once your company has decided to participate please take the following steps: • Complete the Registration Form at http://www.sam-group.com/assessment. • Upon receipt of the Registration Form, SAM will provide you by fax with your passwords. • Please go to the web page http://www.sam-group.com/assessment and log in using your company’s login ID and password. • In the password protected area you will have access to your company specific 2009 questionnaire. • Please carefully read the guidelines posted on the web page on how to use the web-based questionnaire.
