How do I start a Sickness & Accident (S&A) Claim?
You should contact the National Benefit Center as soon as possible at 1-866-DELPHI4 (1-866-335-7444), so that a brief interview can be conducted. It will be helpful and expedite the interview process, if you have the following information available prior to calling in your claim: • The last day you physically worked. • The 1st day you became disabled. • The diagnosis or condition that is preventing you from working. • Your Supervisor’s name and telephone number (plant security number will work). • All dates of treatment from when your disability started after your last day of work including any hospitalizations, surgical procedures, tests that were performed, and special treatments (chemotherapy/radiation/physical therapy). • The name of any and all institutions where you have received treatment. • The full name of all attending physicians, specialty, address, and phone number. • A list of any and all medications with dosage, and how often they are taken. • Restriction information, if
