• Provide the College with the following items: • Written notification (in writing or via email) at least two months prior to your return. • A detailed description of your work history, including photocopies of documents detailing qualifications you obtained since leaving Alberta. • Certificate(s) of Standing from each jurisdiction you practiced or trained in since you left Alberta, dated within 90 days of your start date. Please ensure certificates are issued directly to the CPSA, from each jurisdiction. • Your exact day of return. • Your Alberta contact information using the Notification of Change form. and • Payment of applicable fees*. • *Please contact the Registration department for more information.
