Go to the Parking & Transportation Services (PTS) web site, www.uky.edu/Parking/services-apply-for-permit.html; or b) Apply in person at the Parking Services main office, located on the ground floor of Parking Structure #6, at the intersection of Virginia Avenue & Press Avenue; or c) Complete the application and mail to Parking Services, 721 Press Avenue, Parking Structure #6, speed sort 0571. Application cards will be made available at the UK HealthCare Good Samaritan Hospital.
