Please provide the following information: your e-mail address, user ID, the survey you need to add, the Entity (Utility, Operator, or Preparer) ID and/or Facility (Plant) ID that you will be reporting. (Note: in the event that you need to file for multiple entities or facilities, please be sure to provide the IDs for all of the entities or facilities.
