Please complete the Mental Health Care Services Request for Additional Visits (MHCSR). This request can be submitted, before the patient’s last visit, on the secure Provider Web site by logging on to www.tuftshealthplan.com/providers , or they can be submitted electronically using the Interactive Voice Response System (IVR) by calling 800-208-9565.
