A. If the crash was investigated by an officer of the Ohio State Highway Patrol, a crash report can be obtained online or by completing and mailing the request form (along with the required fee). You can visit the Ohio State Highway Patrol’s website at http://www.statepatrol.ohio.gov/crash.htm to either submit an online request or print out a mail-in form. You can also call the Ohio State Highway Patrol at 614-752-1575.
