How do I requalify for the waiver?
A. Before your initial 12-month waiver ends, you must requalify to extend the waiver for another 12-month period. To requalify, you must: • Continue to participate in the BlueCross BlueShield of S.C. Health Management program. • Take a follow-up survey: the BlueCross BlueShield of S.C. online TRALE Personal Health Assessment or other assessment created by BlueCross BlueShield of S.C. • Satisfy the criteria you originally satisfied for your condition.
