Providers report the adverse event through the Adverse Event Tracking Package (ART) in CPRS and also through the VA Adverse Drug Event System (VA ADERS). Providers have direct access to CPRS. The Chief of Pharmacy (or designee) at every facility inputs adverse reactions into VA ADERS for drugs and vaccines. A Vaccine Adverse Event Reporting System (VAERS) form for all vaccines should be submitted anytime an adverse event occurs. Occupational health should also use this reporting structure.
