How do I report a violation of ARDMS Discipline Standards?
All complaints must be submitted in writing, preferably including a completed Discipline Complaint Form and must include documented evidence of the violation. Please ensure to provide the information with as much detail as possible. Complaints may also be submitted via fax to (301) 738-0312 or submitted via e-mail using the Contact Us section of the ARDMS website, using the Disciplinary Matters contact option.
