In order to report any of the above changes, you must complete an Information Change Request Form. This form can be filled out online. If you need to report any of these changes, click here to be directed to the appropriate form. The form can also be printed, filled out and faxed (225-765-2690) or mailed to the State Licensing Board for Contractors at 2525 Quail Drive, Baton Rouge, LA 70808.
