How do I register for eStatements?
To sign up sign up for eStatements, log on to online banking. After entering your Access ID and submitting your Password, you will be taken to Emprise Bank’s Welcome Page. In the center of the page you will see information about our eStatements and a place that says “Click here to Sign Up”. Click anywhere on the ad and follow the instructions. By selecting this, you will begin the process of signing up for eStatements. This service discontinues your statement that normally arrives by mail. You will instead receive an email from Emprise Bank notifying you that your statement is available online. The statement available online is the same document that would have been mailed to you. If you have not enrolled into Emprise Bank Online Banking, please contact Customer Service and we will be happy to provide you an initial User Code and Password. • Type http://www.hendricksregional.org/estatements into your web browser. • Make sure you have your Hendricks Regional Hospital paper statement in hand. • Click on the “Register” link on the right-hand menu. • Acknowledge that you have read and understand the Disclosure and User Agreement by clicking the “I Agree” button. • Authenticate yourself by providing personal information using the following required items from your printed statement: • Guarantor First Name • Guarantor Last Name • Zip Code (ex. 23456) • Patient Account Number • Enter the security Access Code string exactly as it is shown. • Click the “Next” button to securely submit your information. • Create Account Information for logging on. • Print off your confirmation page for future reference. NOTICE: Your account is not active at this point. You will receive an email to continue registration. This step assists us in maintaining accurate and secure registrations.
