How do I register for a myfife card?
• If you do not already have a myfife card from high school then start by completing the on-line form at www.fifedirect.org.uk or calling 08451 55 00 66. An appointment will then be set up for you at your nearest local services office to complete the registration. When you attend your appointment you will need to take the following: • Photographic proof of who you are i.e passport or new photographic driving licence or existing National Entitlement Card (NEC)/PASS card. • Proof that you live in Fife i.e bank statement, letter from school or college, letter from your health authority. link to library of proofs If you are unable to provide photographic proof then please contact, Elaine Johnson at the Dialogue Youth Team on 01383 602345 for advice.
