Staff must complete a Add/Request Form and submit it to the Office of Professional Development no later than 2 weeks after the session. Add/Request Forms with evidence of course completion (certificate of completion, conference programs, agendas etc.) must be signed by principals (campus staff) and supervisors (central office staff).
