How do I receive CME credit for a CD/DVD series?

If you have purchased a CD/DVD series, there is a URL located on the back of the CD/DVD cover on the bottom left hand corner that will take you to an online test. Type the URL in your web browser to get to the online test, fill in the information, complete the test, and click “submit” when you’re finished. The test is then received, confirmed, and graded. The CME certificate is sent to the e-mail address provided. How do I order extra CME tests for a CD/DVD series? To order additional CD/DVD CME tests you will need the name of the original purchaser.
