You can order your license plate voucher on our web site, or complete our license plate order form (PDF) and mail it to American Military Family. (NOTE: A $25.00 fee is required for each specialty plate purchased. Please complete one form for each vehicle on which you wish to purchase and apply a specialty plate). AMF is offering a campaign for anyone who would like to purchase a specialty plate for a member of the Colorado military. AMF will GUARANTEE that the $25.00 certificate will be presented to a military member. OR, please consider purchasing a gift for a friend, neighbor or someone you know who is currently serving in the Colorado military. *Please note: the name on certificate must be the registered owner of the vehicle.
