The travel consultant at agents.brazilbookers.com will send you the credit card form as an attachment to the email in which your booking was originally confirmed. If not, you may contact your travel consultant by email or phone and ask him/her to resend it to you. If you prefer, you may download it yourself from our website. Just go to the very bottom part of any web page and click on “download form”. Follow the instructions and you will be able to download our credit card form in either Word for Windows format or as a PDF Acrobat Reader file.
