How do I obtain the correct MUST Drug Testing Custody and Control forms (COC)?
A. COC forms will no longer be distributed directly. COC forms will be held by each collection site and will not be available to employers or others who authorize tests. Instead of a COC form, the employee is to be given an Authorization Form obtained from the MUST website. All drug tests will be authorized on line by your MUST account report manager. No other custody and control forms are accepted by the MUST Safety Program. Drug testing completed via other programs not having reciprocal agreement with MUST will not be recognized.
