Judicial review of a revocation decision may be obtained by Writ of Certiorari in the county in which the client was last convicted of an offense for which he or she was on supervision. See Wis. Stats. 801.50(5). Any action seeking a remedy available by Certiorari made on behalf of a prisoner as that term is used in Wis. Stats. 801.02(7) must be commenced within 45 days of the decision to be reviewed. See Wis. Stats. 893.735. The Petition for Writ of Certiorari and the Writ of Certiorari should name the Division Administrator as the respondent and should be served on the Division of Hearings and Appeals at: 5005 University Avenue, Suite 201 Madison, WI 53705-5400 A copy of the petition and writ should also be sent to: Department of Corrections Office of Legal Counsel 149 East Wilson Street Madison, WI 53707-7925 The Division will prepare and file the record with the court only upon receipt of the court order.
