If you currently receive EFT/Direct Deposit for your normal paychecks and you now have a Sickness & Accident (S&A) claim, your benefits will automatically be EFT/Direct Deposit. If you are receiving Sickness & Accident (S&A) Benefits and would like to set up EFT You should contact your personnel department to obtain an EFT/Direct Deposit authorization form. Please mail the completed form to: Processing Center CorPay Solutions, Inc. Mail Code: 000-NPC-002 4600 Rockside Road, Suite 200 Independence, OH 44131 Important: You must mail the original EFT/Direct Deposit authorization form.
