How do I obtain Disabled license plates or placards?
An Application for Disabled Person Identification Placard and /or Disabled Person License Plate (Form VTR-214) must be completed and notarized by a physician indicating the type of disability as mobility or non-mobility. A physician may supply a prescription in lieu of the notorization. An applicant may obtain a combination of two placards, or one placard and one set of plates.
