How do I obtain applications for sprinkler, range hood, and chemical contractors and associated certificate(s) of competency?
To obtain a contractors packet, you may send a written request to the Department of Housing, Buildings and Construction, Fire Protection Systems, 101 Sea Hero Road, Suite 100, Frankfort, KY 40601. The request may also be faxed to 502-573-1598 attn: Linda, or e-mail Linda.Tracy@ky.gov. Be sure to specify the type of contractor’s packet you require and provide a good mailing address as the packets are not faxed or e-mailed back to the requesting individuals.
