Just submit an Electronic Form available through the AIS homepage using the Electronic Forms (E-Forms) link: http://www.siu.edu/~ais/eforms.htm. The AIS forms and instructions are in the Carbondale Campus area under the “Accounting Services” link. Forms available include; Request for Change in Existing User AIS Responsibilities; Request for New User Access; Request New Budget Purpose or Unit / Change in Budget Purpose; Temporary Delegation of Fiscal Officer Authority.
