How do I obtain a RMA (Return Merchandise Authorization) number?

OR 2) FAX: You can fax us the RMA request form to (801) 421-9919. Click here to download the RMA Request form. Once we receive your fax, we will FAX back your RMA number and return mailing instructions. RMA numbers are valid for 10 days after the date of request. To ensure prompt processing, please send returns to the address provided below along with your RMA number within 10 days of the RMA request.
