Complete the following Life Sciences Key Action Request Form: Life Sciences Key Action Form |PDF| Fill out the top portion including your 9–digit ID number. Specify building and room number for keys requested. If requesting a key to a laboratory or a special research facility you will need to obtain the PI/Manager’s signature. Bring your signed form to Linda Zarroli in LSE 237. Keys may need to be ordered which will take a minimum of 48 hours before they will be ready for pick-up.
