Only your veterinarian is authorized to complete and sign a Health Certificate or CVI. Your veterinarian must contact RI DEM Division of Agriculture Animal Health Section for Small Animal, Equine & Large Animal Health Certificates. Equine Infectious Anemia forms are provided by USDA Veterinary Services.
