Then complete the following forms – • Application for a Hackney Carriage and/or Private Hire Driver’s Licence. • Medical Certificate Associated with Application for a Licence to Drive Hackney Carriage or Private Hire Vehicles (this must be completed by a qualified General Practitioner, for which there is usually a charge). • Criminal Records Bureau Check. We require all prospective drivers of Hackney Carriage and Private Hire vehicles to undertake a Criminal Records Check for which there is a charge of £36.00. This check is undertaken every 3 years. • DVLA form. Fee for 12 months including badge, licence and driver’s test – £49.00 for a Hackney Carriage or a Private Hire and £71.00 for a joint licence.
