How do I obtain a First Priority Health Non-Par Referral?
Fill out the FPH Outpatient Non-Participating Provider Request Form and fax the form to the number listed on the form. They will contact you shortly with the Non-Par Number. The Non-Par must be filed prior to or on the date of service to qualify. Please send the specimen to us the same day. If you are NaviNet enabled, you can apply right on line for the Non-Par.
