How do I obtain a copy of my original applicantion file?
• You will need to contact the board at the information below and request a copy of your application. The fee for each page of your application is $.10. If your application is less than $10 it will mailed to you immediately. If the cost is more than $10 then you will have to pay the fee before your application will be mailed out.
