How do I obtain a copy of an Emergency Medical Service Run Report?
If the EMS run report is for yourself or your minor dependent, a copy of the report can be obtained by stopping by the Delhi Township Fire Department Fire Headquarters located at 697 Neeb Road. Due to HIPAA law, there are forms that must be filled out and you must provide a picture ID as proof of identity. Once the paperwork is filled out and approved, a copy of the report will be released to you. There is a fee of $5.00 for each report requested. Because of the HIPAA law, you cannot obtain copies of reports for relatives, friends, etc.
