How do I obtain a copy of a check and/or EOP that was previously issued?
If the check came from ProMedCo or one of our five IPAs, you should contact the provider representative for your region to request a copy of an EOB or a check. However, if the check stock says WellCare, you should contact WellCare directly at 800-421-1755 to request copies of missing checks or EOBs. If you are unsure please contact your provider representative.
