There is currently a fee of $15 dollars for this request. Please send a check in the amount of $15 to BOREA with your request form for your license history to: BOREA Attn: Licensing 1560 Broadway, Suite 925 Denver, CO 80202 Please include your name, license number and tell us what state to send the history to. If you want it to be sent directly to you, please let us know. The certificate will be processed in 3 to 7 business days.
