Any supervisory or non-supervisory employee may nominate an individual or a unit for the Excellence in Preventive Medicine awards. The award narrative should describe the impact of the contribution, which would qualify the individual or unit for the award. The submission format is narrative and will not exceed 1 page using Times New Roman font at 12 pitch/font size. This can be in the form of a memo with concurrence by your chain of command.
