You will need to get a Health practitioner (doctor, hospital, clinic, emergency room, or equivalent) to verify your illness using University of Waterloo’s official Verification of Illness form. Other doctor’s/clinics/hospital or similar notes will not be accepted. If you do not have UW’s Verification of Illness form with you at the time you are seen by the Health practitioner, you will need to go back and have them complete and sign the form to include the severity of the illness and the duration. This official verification is necessary before any action can be taken. For other extenuating circumstances that are not illness related, for which the Verification of Illness form is not appropriate, official documented proof that something has occurred will be required (plane tickets, court documents, death certificates, counsellor letters for example).
