At this time these forms are not downloadable from these websites, so you will have to call in or telephone to request copies. If your complaint relates to discrimination on the grounds of religious belief or political opinion you must complete the blue claim form. The yellow form must be completed for all other types of complaint. From October 1 2005 the tribunal will only accept applications on the correct form. You do not have to pay a fee or charge in order to lodge a claim at the tribunal.
