Medica-appointed agents can get information online by going to Medica.com>Brokers>Commercial Broker>Sales Material. Choose Applications and Forms and then select Minnesota, Enrollment and Changes and MCHA for: MN – (MCHA Medicare) Minnesota Comprehensive Health Association Medicare Application.
